We all agree the bacteria living in your mouth, especially in between our teeth (see “Floss2BFree“) gets excited by sugars and their excretory byproduct “poop” is acidic and will demineralize enamel over time and then decay develops.

It’s the low PH or an Acidic environment that’s the problem.

But we also need to realize that carbonation breaks down to carbonic acid and water.

So…. Even plain water with a fizz, ie Perrier is bad – especially for roots with recession causing erosion.

It’s NOT just the sugar it’s the Bubbles.

We all suspect soda pop is bad for us but so is sparkling water. So when your server says still or bubbles? Say still!

Neutralize your mouth after consuming seemingly innocent things like tea, (tannic acid) coffee, orange and fruit juices, and vinegar, (all citric acid), beer and the worst of all carbonated Monster and Rockstar energy Drinks – bubbles, and caffeine (which causes clenching) with a water chaser and do it by vigorously drawing still water thru your teeth – repeatedly- and swallow (good hydration is essential) until it tastes like you not what you ate or drank.

The byproduct of most bleaching agents (Carbamide Peroxide) is water and Carbonic Acid  – just like diet soda! So…. Don’t over bleach your teeth or they will go “frosty” and stain more easily and be weakened!

Carbonation robs your muscles of calcium, causing an imbalance away from relaxation and shifts us toward contraction, tension and “leg cramps”- that’s bad. It will also cause bruxers to brux. Any additional teeth grinding is very bad (see the “The Day Guard“).

So rethink the tiny bubbles and acidity – it’s basic!

The absolute best way to avoid getting bad news from your dentist … is so simple – just FLOSS! Rick Glassman, DDS

Lets see – check up X-rays or Bitewings are specifically taken looking primarily for decay, attacking tooth structure between the teeth, that is likely unable to be seen during a visual clinical exam and often difficult to diagnose in X-rays in many situations – like teeth that are crowded.    Patients understandably resist having X-rays taken and often these cavities lurking and growing in between un-flossed teeth ( ie inter-proximal decay) are often quite deep,  involving many teeth ( back to back decay), threaten the nerves in teeth and compromise tooth structure that makes teeth prone to breakage,  pain,  food impaction trauma, and eventually tooth loss by extraction. All from not Flossing!

Those of us not flossing, will look forward to gum disease over time – as we age. That’s just a fact – the way it is – guaranteed – no doubt about it … and still patients fail to connect the dots that flossing changes all that – flossing is better than any dental insurance available – and patients should understand dental problems that come from not flossing far outweigh problems from not brushing correctly or infrequent brushing .

Really? Who doesn’t brush their teeth during the course of a week? Yet so many of us never floss. I never met  anyone who said, “I never brush my teeth” but I’ve met many patients that will sheepishly admit to not flossing at all! And patients are foolishly misguided to believe  that seeing the Hygienist an extra time or two during the year will replace regular flossing the other 361 days each year. It won’t!

Those of us not flossing, will look forward to gum disease over time  – as we age. That’s just a fact – the way it is – guaranteed – no doubt about it … Rick Glassman, DDS

Lets put it this way, it would be nice to hear, “I floss every day and I can’t go to bed or greet the day without flossing first” or “I feel guilty if I miss flossing each and every day.” Well, I do hear it! I hear it because I preach the gospel and relevance of Flossing. I tell my patients I do it every day, you should too. I inform them that I haven’t missed a day in years and demonstrate by flossing my own teeth quickly and efficiently with my eyes closed … I make the room quiet so they can “hear” what firm, healthy, regularly flossed gums sound like. I tell them how empowering it is to see all the bleeding stop after only two straight weeks of daily flossing… gone… done… over and out, and how I giggle as I remind myself that I will not end up needing gum surgery (no matter how much it thrills the numerous practiced periodontists available for us all). I will never need to retreat my beautifully previously restored teeth (the bonus of not flossing as a teenager). And at the same time I know I can spend hours of time relaxing in a hammock instead of lying prone while gripping the arm of a dental chair.

The point is . . .

We should make flossing proficiency mandatory in grade school and reward it with extra credit toward college admission, not to mention think about the money saved on unneeded dental care that could be applied to college savings or better still a sensible government approved incentive in the form of a tax write off for ZERO cavity check ups.

Honestly when I review the schedule each morning and realize the profound majority of daily work will be treating is some kind of problem between some poor back teeth due to not flossing, it sickens me!

These eager patients are ready to tackle the dental problem, perhaps they’re even ready to take out a loan, ask their parents for financial aid, ready to spend their just gotten tax refund so they can do what? … pony up for that crown and or cast post, and or root canal treatment,  and or gum surgery? All for one tooth. And I tell them that this effort too shall fail and likely will fail badly if the new addition is not flossed! I remind them, “the best dental work is second best to the original … the same tooth that decayed from not flossing in the first place.”

Patients need to be encouraged to get better at flossing and be educated as to what products work … using a tooth pick or water-pik can help but will not eliminate the risk of tooth decay like plain old dental floss will. If you hear, “I don’t like the mess” then tell them to do it in the shower. Praise their improvements when seen by having patients demonstrate floss competency at each appointment! No dentist wants to retreat their Patients dental work that they installed. So knowing that flossing adds years of service to your dentistry – think about giving a “flossing discount”  for good probing without bleeding at their Hygeine appointments! We encourage night guards for grinding, thus protecting that expensive dental work. How about driving home the message about Flossing! Speaking of which, studies reveal that people that floss regularly live an average of 6 years longer! Anyone else given you an extra 6 lately?

Think about it. Be an example to your kids. Let your dentist figure out other less invasive ways to improve teeth with needs that still exist for many of us, even the flossers amongst us, that still have so many teeth that were wrecked by decay and larger fillings when we were kids… Kids that never heard about this thing called Floss!

Have you noticed your teeth are either biting pressure sensitive or cold sensitive? Have you been told that you have a cracked tooth and require a crown? Have you noticed that your teeth feel loose or are your gums bleeding more than usual? Are you getting headaches more frequently?  Do you have any porcelain crowns that hurt? Have you noticed you are biting down and feeling more contact on your front teeth than your back teeth? If so, the good news is: Your new symptoms may be related to what we call occlusion.

Equillibration

Occlusion is the way your teeth meet properly. Malocclusion is an improper bite. The reshaping of the biting surface of natural teeth or porcelain crowns is called an Equillibration. The reshaping mentioned for biting strait down is called establishing proper “centric relation” and the proper reshaping for side to side, or chewing motion is called correction of “lateral interference.” Skillful equillibration of teeth is similar to both fine tuning an engine or balancing and rotating ones tires – the advantage being your teeth when equillibrated or fine tuned as needed can spare you unnecessary and often expensive dental work like balancing your tires on your car can prolong your tire life and likely avoid a “blow out.” All equillibration is preformed painlessly without the need for anesthetic. Also malocclusion contributes to recession of your gums and must be remedied prior to any corrective periodontal measures, ie “grafting, or “PST.”

EquillibrationAll of the symptoms mentioned above can be related to a malocclusion. All porcelain crowns wear down more slowly than natural teeth and eventually all porcelain crowns require an equillibration. All teeth have a pressure sensitive ligament called the “PDL.” The PDL reflexively opens your mouth and instantly hurts when a tooth is acutely traumatized. For example: unexpectedly biting down on a cherry pit or popcorn kernel. It hurts, we don’t think about it and we automatically open our mouth – the “Yin” of the PDL. Conversely as a porcelain crown ages it will begin to hit prematurely and cause the “Yang” of the PDL to attempt to correct this malocclusion. In other words, we will grind our teeth at night subconsciously in an attempt by the PDL to fix our bite. During this process our teeth can become loose or painful as our muscles of biting “mastication” become abnormally enlarged, powerful, inflamed and/or “hypertonic.” These muscles can be tender to touch or palpation, and “trigger point” referred headache pain can result. All of the above can be simply corrected by equillibration once appropriately evaluated. The protocol to equillibrate more than one tooth predictably may require an analysis of your bite prior to equilibration, facilitated by study models, and/or a nightguard or splint. The goal of splint therapy is both to improve the biting surface of maloccluded teeth, eliminate lateral interference, thereby allowing sore and enlarged chewing muscles to relax and begin the process of healing of TMJ dysfunction.

Bruxism, Nightguards and Botox

EquillibrationThe good news for people grinding their teeth (bruxism), day or night that have likely headache pain and muscle tenderness, or painful teeth is that as of 2011 the FDA has approved therapeutic injection of Botox for migraine related issues previously mentioned. Botox is an extremely safe and amazingly effective medication to achieve normal muscular activity by relaxing and weakening hyperactive nighttime grinding muscles – one of the major goals of nightguard treatment. This is really exciting news for people who cannot tolerate wearing a nightguard. The deprogramming of abnormal muscular activity typically requires three to six months of wearing a nightguard or can be simply achieved in two weeks after receiving therapeutic injection of Botox. Botox relaxes the muscles affected and can allow for proper equillibration, minimize or negate the time typically spent wearing or “not wearing” a night guard. Dr. Glassman, a member of the American Academy of Facial Esthetics has been extensively trained and is licensed and certified to preform these therapeutic injections. Dr. Glassman has seen amazing results with both nightguard “splint” therapy, therapeutic injections and/or equillibration often completely eliminating the causes of bruxism, migraine, tension headache, and painful teeth.

The U.S. Food and Drug Administration today approved Botox injection (onabotulinumtoxinA) to prevent headaches in adult patients with chronic migraine. Chronic migraine is defined as having a history of migraine and experiencing a headache on most days of the month.

“Chronic migraine is one of the most disabling forms of headache,” said Russell Katz, M.D., director of the Division of Neurology Products in the FDA’s Center for Drug Evaluation and Research. “Patients with chronic migraine experience a headache more than 14 days of the month. This condition can greatly affect family, work, and social life, so it is important to have a variety of effective treatment options available.”

(WESTLAKE VILLAGE – January 14, 2014)Chao Pinhole Gum Rejuvenation™, is a new minimally invasive treatment for receding gums without the invasive surgery and recovery time required with gum grafting. Dr. Glassman is among the first group dentists and periodontists to be trained in the new technique, invented by Los Angeles dentist Dr. John Chao. The treatment is also known as the Chao Pinhole Surgical Technique™.

“The conventional treatment for receding gums is gum grafting surgery, an invasive procedure in which a section of gum tissue must be cut out of the palate, then sewn on to the affected area,” says Glassman. “Because of the surgery and recovery time, dentists will usually treat only one or two teeth, then wait for the patient to heal before treating additional teeth. For a patient with several recessions, the treatment process could take months or even years.”

Chao Pinhole Gum Rejuvenation™ takes just a few minutes per tooth treated and most importantly multiple teeth may be treated at the same time, allowing patients to have healthy, normal-looking gums with a minimum of inconvenience.

Receding gums can affect people of all ages. Symptoms can include sensitivity to cold temperatures and if left untreated gum recession can lead to eventual tooth loss. Receding gums can be caused by excessive tooth brushing, gum disease or the normal aging process. “Many people are unaware of the condition until their teeth become sensitive while others are concerned by the unsightly appearance of their gums,” says

The Chao Pinhole Gum Rejuvenation™ treatment process:

The area of gum recession is numbed with a local anesthetic. Depending on the number of recessions to be treated one or two small entry points about the size of the tip of a ballpoint pen are made in the gum area near the tooth or teeth to be treated.

A small, specially designed dental instrument is inserted into the entry point to loosen the gum tissues and move them back into a normal, healthy position. Then tiny collagen strips are placed through the entry point under the gums. The collagen helps to keep the gums in place during the healing process. The treatment takes about 20 minutes to an hour or so, depending on the number of teeth treated. The entry site quickly heals and the pinhole is virtually undetectable the next day, with no downtime needed for healing. Some patients may experience mild swelling, which dissipates in a few days.

A 33-month study of 43 patients with 121 gum recessions using the Pinhole Surgical Technique™ was published in the October 2012, issue of The International Journal of Periodontics and Restorative Dentistry. The results of this study were as successful as traditional gum grafting procedures which require cutting and sutures, while patients who underwent the Pinhole Surgical Technique™ reported virtually no pain, no bleeding and high satisfaction with a rapid transformation of the defective gum line. Goal: Patient had left front crown fractured off at gumline while overseas and required root canal treatment for future Post/Core.  Patient returned to me for comprehensive treatment that included a custom cast post and core followed by splinted crowns.

Solution: A free standing endodontically treated front tooth often requires splinting to an adjacent tooth.  In this case the adjacent tooth had been previously crowned and was vital (w/o root canal).

The patients goal was to improve the appearance of his smile without compromising the space between his front teeth.  Note the upper right bicuspids, just behind the upper right “eye-tooth” were set back and appeared darker as a result of sleeping with his palm under his right cheek when he was a child.  This is a perfect example of how analyzing pre-operative photographs during the treatment planning phase enables the proper evaluation of this issue.  As a result, this case was treated with two Porcelain Veneers on the upper right bicuspids (not the upper left) and four Porcelain Veneers on the front four teeth, for a total of six Porcelain Veneers.  No drilling.  No shots.

This patient’s goal was to improve the form and color of his upper front teeth.  The use of Porcelain Veneers achieves this goal and at the same time, improved his bite.  Please note the integrity and identity of his original smile has been preserved, especially as it pertains to the two teeth on either side of his front two teeth.  This subtle flaring of the lateral incisors is called “Class II Division II Occlusion.”

This case involved fixing two front teeth for an 11-year old child after a swimming pool mis-calculation.  With the improved strength of the porcelain veneer product (E-Max), this case was solved without needing to use crowns, prevented the need for a root-canal and prevented the need to remove the natural teeth.  Here the mid-line space was reproduced as it is age-appropriate and this space will close as the patient grows older.

Here the treatment goal was to diminish the slanted and worn appearance of the teeth.  Using the no-prep technique, 7 of the 8 front teeth were veneered with careful attention to the width and length of the veneers, which also then restored ‘cuspid rise’, diminishing a habit of grinding teeth and ultimately restoring the bite.  This particular issue is ideally suited for the no-prep technique.

This patient came to me with existing porcelain veneers on her front two teeth, which we placed 28 years ago and were recently chipped by a collision with her dog. After experiencing the durability of her initial no-prep veneers, she decided to add additional veneers to her overall smile, as well as bleaching her natural teeth to remove stains.  Of the ten teeth in her smile, only 7 are no-prep porcelain veneers.  No drilling.  No shots. Hint – her smile has been widened to reveal a wider, brighter smile.
